SOCIAL WORKER (FAMILY SUPPORT)
Full time 37.5 hours, 5 days per week
GBP25,476.00 (Probationary rate) - GBP28,306.68
An exciting opportunity has arisen for a qualified social working to join the Family Support Team at the Hospice. The role operates across the hospice services, predominantly with the Inpatient Unit team.
The principal function of the role is to support and advocate on behalf of individuals and their families within specialist palliative care who have complex health and social care needs.

- Degree in SW, DipSW, CQSW or CSS
- Experience of working in an organisation providing care services with adults/older people and an understanding of the Care Act and partnership working with service users and carers
- Experience and skills in assessing need and individualised care planning with individuals and their families
- Experience in working effectively with other agencies and professionals
- Flexible attitude
- IT literate, experience in using manual and computer systems for record keeping.
- Have a positive, caring and approachable manner
