St Luke's Specialist Social Worker

37.50 hours per week

Band 6 Salary range £26,392-£35,753 pa pro rata exact point dependant on qualifications and experience

St Luke's Hospice is the major provider of specialist palliative care to the people of Plymouth, West Devon and East Cornwall. It has a community team, an inpatient unit and a clinical education department.

We are looking for an experienced social worker to provide support and advice to patients with progressive life threatening disease and their carers to ensure consistent access of patient and families to social care and psychosocial support. The post holder also supports the development of in house social care services or facilitates signposting and referral to and collaboration with external agencies as appropriate. The primary focus of the role is to provide direct social work and psychosocial intervention in relation to complex issues such as vulnerable adults and children, mental capacity assessments and best interest care planning including Deprivation of Liberty Orders. The post holder will be based at our Brooklands site in Crownhill, Plymouth.

The role will include in reach support in to the in-patient unit as well as contributing to multidisciplinary team decision making. We have a well established integrated service and require a dedicated and compassionate social worker with post qualification experience and a commitment and passion for teaching others.
