In Petersfield we currently have an amazing opportunity to join our team as a Care Coordinator! The care coordinator is responsible for the continuity of care for all of our customers.  This vital role has a huge impact on the lives of many people who rely on our care and support. Customers tell us that what really matters to them is having a stable team of caring and professional care workers whom they can rely upon. Working alongside the care supervisor, the care coordinator will be responsible for making this happen. The ability to problem solve and think clearly when under pressure is essential for this important role in our company. You will use your excellent planning, organisational and IT skills to make sure that each customer receives the care and support that they expect on time. This involves:

•Allocating care workers with the right skills and experience to deliver the best quality care and support to each customer

•Organising care worker rotas to minimise changes to the service and travel between customer’s homes

•Responding efficiently to day to day changes in the care and support packages

•Covering visits when they are unable to be covered by the field team

•Providing excellent customer service

•Building excellent relationships with our current care assistants

In Return you will get:

•A salary circa £19,500.00 per annum

The role offers the opportunity for developing positive working relationships with customers, their families and the Bluebird Care team. You will provide a crucial link between each customer and their care worker. You will often deal with queries from customers about their care and support. This requires a sensitive understanding of customer needs and a supportive telephone manner.

The care coordinator must also be clear about the best way to work with care worker schedules to maintain continuity in the delivery of care. You will need to build strong relationships with the care team so you can call upon extra cover when needed.

Care workers may work over the weekend or evenings so the post holder must be willing to be part of the on call emergency team rota; However regular office hours are 8:30am to 17:00 Monday to Friday.
