St Luke's Specialist Nurses work in teams based in the Community, and we currently have a vacancy for a Band 6 post-holder.

We are seeking an enthusiastic and highly motivated RGN to join the multi-disciplinary team in the provision of specialist palliative care to patients and their families. The expectation is that the experienced post-holder will join the rotation of staff across care settings including the community team/acute hospital team and will participate in the on-call arrangements of each setting as appropriate.

The post holder will need to be able to work autonomously and be responsible for complex holistic assessments and planning care to meet patient's practical, psychological, spiritual and social needs. They will work closely with primary and secondary care professionals and Hospice colleagues, and good communication and team working skills are essential. Applicants should have palliative care experience and be in possession of, or working towards, a relevant degree.
