If you are then we may have the perfect job for you. The role involves supervising support workers and providing out of hours 'on call' cover as required as part of an 'on call' rota. Responsibility for setting up new service user relationships and on-going monitoring and assessment. To provide assistance with personal care and other daily living tasks to disabled people within their own homes when required. To work in partnership with service users encouraging choice, participation and motivation, adhering at all times to the values of the organisation
You will support people with all aspects of daily living, ensuring that their independence is preserved and developed.

Key Responsibilities to include:- Team Leader Social Care (Community), Sheffield
- Carry out day-to-day supervision to monitor the quality of support provided by support workers
- Undertake formal supervision with support workers as required by Leonard Cheshire Disability and the domiciliary care standards
- Conduct regular service user monitoring visits to assess appropriateness of the support package, and to make recommendations
- Carry out risk assessments and moving & handling risk assessments in service user's homes, and other places where care is provided
- participate in the services 'on call' rota
- Undertake additional responsibilities as requested by the Service Manager following the successful completion of specific training and personal skills development
- Assist with Support Coordination tasks as required
Key Experience preferred for the role:- Team Leader Social Care (Community),, Sheffield
Previous experience in a Senior/Supervision or Team leader role from a Social care background highly desirable
- Minimum NVQ 2 in Care/ Diploma in social care or Level 3 highly desirable
- Manual handling qualification/certificate desirable
- A full driving license you can use in the UK
- Ability to work flexible hours when required, and to travel to fulfil the objectives of the job essential
- Good basic literacy and numeracy
- To be willing to undertake required training (including updates), sometimes off site and outside normal working hours.
