TEAM LEADER/HEALTH CARE ADVISOR

 

JOB DESCRIPTION

 

  Working as a Team Leader/Health Care Advisor within Madog Care Group, your duties will be as follows:-

 

  • To provide team leadership and supervision of staff.

 

  • Assessing training needs of staff and arranging / conducting in day to day management of the home.

 

  • Assisting in care planning of the residents.

 

  • Continuously assess and monitor the dependency of the residents to determine the provision of adequate staffing levels.

 

  • Carrying out audits and surveys and assisting with quality assurance.

 

  • To support the Management team in delivering high standards of care and to develop high standards of care.

 

  • To liaise with health professionals upon their visits to the residents.

 

  • To be aware of the Home’s Philosophy of Care and apply its content to your everyday duties.

 

  • To give care to each Resident which will contribute to a homely, happy and comfortable environment in accordance with individual Care Plans.

 

  • To inform the Nurse in charge or the Manager of any sign of illness or change in condition of any Resident.

 

  • To ensure that discipline and correct working practices are maintained in the absence of the Manager or her Deputy.

 

  • To carry out regular documented supervision on all care staff as per supervision matrix.

 

  • To assist with the induction and training of newly recruited staff and to act as their mentor when requested to do so.

 

  • To report to the Manager or Deputy Manager any breaches of Company policy or discipline or any other areas of concern.

 

  • To assist each Resident to maximise their independence within their own personal limitations, having due regard for their privacy and dignity.

 

  • To understand and participate in the key-worker scheme.

 

  • To achieve the standards which have been set and ensure that they are abided by.

 

  • To be aware of disciplinary procedures and what would result in disciplinary action being taken. (Available in Policy Manual)

 

  • To abide by the policy on standards of dress within the home and ensure that other team members do so also.

 

  • To be aware of policies relating to Health and Safety, COSHH, fire prevention, action in the event of a fire and all other policies which relate to the well-being of the Residents and fellow staff members.

 

  • To attend staff training sessions / staff meetings when requested to do so by the Manager or her Deputy, or by the Area Manager or Responsible Individual.

 

  • To assist in housekeeping, laundry and kitchen duties when requested to do so by the Manager or her Deputy, or by the Area Manager or Responsible Individual.

 

  • To encourage and support Residents to take part in social and recreational events and to accompany them on outings if requested to do so by the Management.

 

  • To accompany Residents receiving hospital treatment when requested to do so and to report back to the Nurse in Charge or Manager any information gathered during such appointments.

 

  • To inform the Manager if further training or support is felt necessary in order to competently carry out duties outlined.

 

  • Any other duties that are deemed necessary by the Manager, her Deputy, Area Manager or Responsible Individual.
