Flame Health currently have a post for a Consultant in Care of the Elderly based at a community hospital in the Surrey area. This hospital provides comprehensive geriatric assessment in the Diagnostic and treatment centre, in-patient work managing assessment and rehabilitation of a ward along with community liaison working with local GPs and where necessary undertaking occasional domiciliary consultations.

This position is full time and suitable for an experience medical practitioner with an interest in Elderly care. You will be working seamlessly with the local acute services.

Key Responsibilities:

  • Provide clinical leadership to the MDT responsible for the Consultant's agreed area
  • Have a continuing responsibility for the care of patients and for the proper functioning of the wards
  • Liaise with the General Practitioners, hospital and community specialists and other agencies wherever relevant
  • Undertake the administrative duties associated with the care of patients and the running of the service including the prioritisation of the case load according to need
  • Be responsible with other professional colleagues for the provision of a comprehensive geriatrics service
  • Contribute to the development of clinical expertise for the benefit of patients and the service as a whole
  • Participate in training, teaching and supervision, as applicable with the speciality doctors in the service
  • Active participation in clinical research, audit and continuing professional development will be encouraged
  • Responsible for offering expert advice on clinical issue
  • Participate in the Consultant Appraisal process and fulfil professional requirements for Continuous Professional Development (CPD)
  • Work with the Service Lead to develop the service to meet commissioning intentions
  • Provide an effective and efficient community based outpatient and inpatient service for the comprehensive care of elderly people presenting with complex needs
  • Provide Consultant led comprehensive specialist geriatric services, capable of managing patients in the community avoiding unnecessary admission to the acute trust
  • Provide a service that works in close partnership with other agencies, particularly social services, to ensure that health and social input have a coordinated approach
  • Promote the team approach to service delivery, recognising the contribution of each member. Including participation in CPD of team members and supporting day to day team management
  • Maintain and enhance relationships with other professionals such as Consultant colleagues, General Practitioners, Public Health colleagues, therapists, nurses etc and similarly with agencies such as social services, employment and educational services and voluntary bodies

Candidate requirements

  • Full and unrestricted GMC registration on the GMC
  • Qualified GP or specialist register (Subspecialty)
  • Medical qualification
  • MRCP or equivalent
  • Clinical expertise in specialty
  • Appropriate training as defined by the Royal College of Physicians
  • Demonstrated interest in Age Related Medicine Knowledge of rehabilitation of all ages
  • Ability to select patients for rehabilitation
  • Ability to offer expert geriatric opinion on full range of emergency and routine clinical situations
  • Awareness of research methods and understanding of application of research findings in service development
  • A track record of clinical quality improvement
  • A detailed knowledge of professional regulation and evidence based practice
  • Excellent interpersonal and communication skills
  • Mature influencing; a team player able to work in multidisciplinary teams
  • Self-motivated and a completer finisher
  • Self Management -successfully plans own workload with awareness of needs of others and/or those of the team

Desirable:

  • A practising clinician with over 5 years' experience of providing NHS services
  • A good working knowledge of VCL
  • Higher degree/diploma
  • Experience in community and care home liaison work
  • Additional training and experience in movement disorders
