Social Worker - IRIS team (Intensive Reablement Services)

£29,854 - £34,196 p.a

Location: St Francis day Hospital, St Francis Unit, City Hospital, Hucknall Road, Nottingham NG5 1PB

The Gedling Intensive Recovery Intervention Service (IRIS) is seeking to recruit a suitably qualified and experienced Social Worker to join the team on a permanent basis.

You will be experienced in adult social work and able to demonstrate that they can work in a person centred way while adopting the Adult Social Care Strategy and its emphasis on maximising independence, preventing and delaying the need for care and working in collaboration with the voluntary sector and informal carers.

Core tasks will include assessment, support planning and risk enablement alongside utilising the range of resources available within the community to prevent unnecessary admission to hospital and long term care where possible.

You will be part of a multidisciplinary team which provides services including specialist assessment, active therapy, treatment, and the opportunity for recovery for older adults with mental health issues.

The IRIS Team provide a flexible and creative person centred approach to enable the Service User to engage in services within their own home. As a Social Worker within the team you will complete the full range of assessment and support planning activity including contributing to duty systems and undertaking safeguarding enquiries.

There will be a requirement to carry a complex caseload, be able to organise and prioritise the workload and be highly motivated in order to meet IRIS discharge timescales and respond to changing priorities and demands.

The post is located within a health setting, currently on City Hospital campus, but you will also maintain strong links with Gedling Older Adults Team who will provide professional support.
