HFH Healthcare is a complex healthcare service providing bespoke packages of care to highly dependent adults & children living at home with complex conditions requiring specialist care. Our aim at HFH Healthcare is to provide our clients with care teams of the highest calibre.

We are looking to recruit a dedicated Care Coordinator to work closely within the Care team to ensure client care packages are appropriately staffed and managed. Our in-house clinical team of nurses provide the specific training, however you will need to ensure the care team are appropriately allocated.

You will be managing a case load, being the first point of contact by telephone for the clients and care teams and therefore professionalism is key. This is a complex role in which you need solid management experience to handle the challenges which arise on a daily basis. However it is a rewarding job where you will have the opportunity to make a real difference to the service we provide to our clients.

To be successful you will need to be solution focused, able to deal with a fast paced environment, working under time pressure. You will be responsible for dealing with any changes to the rota ensuring sufficient cover for your client is provided at all times. 

Ideal candidate:

-Healthcare coordinator experience

-Ability to manage a team of healthcare workers

-Experience of managing staff rotas

-Experience of working with a database/CRM system

-Able to work under pressure and work to tight deadlines

-Able to multi-task and be organised

-Resilient and good interpersonal skills

Pay Rates: Annual Salary up to £30,000 pa including allowance for on-call duties

Hours: 37.5 (office based in Wimbledon with duty on-call working on a rota basis).

Annual Leave: 20 days plus bank holidays
