JOB TITLE: GP Facilitator
SALARY & BENEFITS: GBP17,161-GBP19,126 pro rata
WORKING HOURS: 16 hours, FTC until March 2017 (continuation of post dependent on securing further funding)

THE COMPANY: Our client is a registered charity that provides information and support for carers aged 18+ in Gwynedd, Conwy and Anglesey. They provide a wide range of services including individual support through home visits, emergency planning, social events, support groups, health and well-being sessions, outings and a quarterly newsletter. They also assist carers to access grants and benevolent funds.

THE ROLE:Our client is seeking a GP Facilitator to join their team in Anglesey. Within the role, you will facilitate early identification of unpaid carers and young carers by primary care health services and professionals. You will enable them to access support and services within primary care settings, promoting and increasing awareness of carers' issues within communities and within statutory, voluntary and private sector environments.

Key Responsibilities to include:
  • Working with GP surgeries to identify unpaid carers
  • Working with GP surgeries to raise awareness of carers' issues
  • Providing support and information to identified carers by encouraging them to register with the service to receive tailored holistic support
  • Being proactive within the community and finding new and innovative ways of identifying hidden carers
  • Arranging information and awareness raising events within GP surgeries, in particular during Carers Week and Carers Rights Day
  • Attend relevant meetings, training and supervision sessions
  • Keeping appropriate records of work undertaken and providing regular evaluations and reports to the Chief Officer
  • Working within the confines of the Data Protection Act 1998
  • Undertaking any other duties as reasonably requested in line with the needs of the organisation
THE CANDIDATE: The ideal candidate for the GP Facilitator position will hold a relevant Health and Social Care qualification or vocational/suitable work-related experience. You will have experience of using computer based information sources and the internet and experience of monitoring performance and outcomes. A knowledge and understanding of unpaid carers and their needs is essential, alongside an understanding of how GP surgeries are managed and the various roles within each practice. Ability to communicate in both Welsh and English is essential, as well as possession of a clean driving licence and access to your own vehicle.

Key experience essential for the role:
  • Relevant Health and Social Care qualifications or vocational/suitable work-related experience
  • Experience of using computer based information sources and internet
  • Experience of monitoring performance and outcomes and maintaining up to date records (e.g. Excel/database)
  • Knowledge and understanding of unpaid carers and their needs
  • Understanding of how GP surgeries are managed and the various roles within each practice
  • Understanding of how third sector organisations can play an important part in improving the delivery of health and social care
  • Ability to communicate effectively at all levels and strong networking skills
  • Ability to work with accuracy under minimum supervision
  • Ability to communicate through the medium of Welsh and English and the ability to write reports and give presentations bilingually
  • Excellent interpersonal and organisational skills and the ability to initiate work and provide a constructive, problem solving approach to tasks
  • Possess a clean driving licence and have access to own vehicle
  • Be willing to attend out of hours meetings if and when required
Key experience desirable for the role:
  • Experience of working with carers and a knowledge of carers issues
  • Experience of networking and building strong connections across statutory, third and private sector agencies
  • Experience of dealing with and addressing challenging issues
  • Experience of working within the primary care sector
