Carmarthenshire County Council and Hywel Dda University Health Board are seeking a motivated and resourceful leader to the senior integrated post of Locality Manager for the Aman Gwendraeth Locality of Carmarthenshire. The post holder will be accountable for the provision and management of health and social care services to support the needs of the older adult and adults with physical disability of the locality.

The Aman Gwendraeth is a vibrant locality supported by a cluster of 8 GP practices and its Community Resource Team consisting of multi disciplinary professionals and their related services. The post holder will be responsible for driving forward the delivery of the modernisation agenda for primary care and community services for older people and adults with physical disabilities in the Locality along with the GP Lead. This is a senior management post, reporting to the Head of Integrated Services and responsible for ensuring population health and wellbeing and managing the full range of services within the Locality, including staff, budgets, information, commissioning, activity& performance. The successful post holder will be joining a new team at an exciting time where innovative service solutions are being developed on the ground to improve patient experience and where integrated working is becoming a reality not just an aspiration.

Conversational Welsh is required to accomplish this post. Support can be provided on appointment to reach this level.
