The Role:

The successful Assessment & Review Care Coordinators will be responsible for introducing all services to customers, identifying how the organisation can meet the individual’s goals and aspirations to maximise independence. The role includes:

• Working with the individual to draw up a service plan
• Assessment of risk
• Promoting choice and control
• Reviewing services, monitoring progression and completing onward referrals as necessary
• Liaising with other health and social care practitioners e.g. occupational Therapists, Registered Nurses, Community Nurses, local GPs
• Working in an enabling way

The Person:

• Approved Assessor 2
• Candidates need to have the ability to problem solve and make decisions, demonstrating flexibility, adaptability and a positive ‘can do’ attitude.
• Ability to interpret community care assessments into Service Users plans
• The ability to network and forge strong and productive partnerships across the organisation and with external partners is vital, as well as having the ability and vision to support the development of integrated community support services.
• Experience of working within a care environment, creating person-centered service plans and undertaking risk assessments would be an advantage.
• The candidate will be familiar with dealing with sensitive data and have an understanding of Information Governance.
