Assistant Practitioners

Community-based - will need driving licence & access to a car

At Birmingham Community Healthcare NHS Trust, we are looking for Assistant Practitioners to join our community team, supporting the district nursing team in meeting patients’ nursing needs in their own homes.

No two days will be the same, making this an excellent opportunity to take on a higher level of responsibility in the hands-on delivery of quality care and evidence-based intervention for patients with a variety of conditions - and to develop a breadth of clinical skills and experience.

This role will suit people who can provide wide-ranging practical care and take appropriate action to achieve planned aims and objectives for individual clients, within their home, without direct supervision. It involves managing care of patient’s on Clinical Case Manager and District Nursing caseloads and accepting delegated responsibility for the delivery and reassessment of individual plans of care.

Areas of care you could help to deliver include:

  • Venous leg ulcer management
  • Medication administration, prompting and support
  • Assessment of pressure ulcer risk and management of pressure ulcers 
  • Wound management
  • Assessing a client’s nutritional status using the MUST tool
  • Supporting the management of patients with respiratory conditions
  • Undertaking, under delegation, the administration of B12 injection
  • Undertaking, under delegation, the administration of Glargine injection
  • Undertaking, under delegation administration of Clexane injections,
  • Undertaking under delegation , female re-catheterisation

We would expect you to have completed a foundation degree in health and social care, or to hold a suitable alternative qualification. You should have experience in a community or hospital setting - including undertaking baseline observations and tissue viability - or similar appropriate clinical experience.

Able to work independently, following clear care pathways and guidelines, you are also capable of using your initiative to challenge or escalate issues relating to practice, in the best interest of the patient. Given the community-based nature of the role, it is essential that you have a driving licence and access to a car.
