Social Care Solutions provides supported living services for adults with a learning disability, complex needs, autistic spectrum conditions, mental health needs and/or challenging behaviours. We support people individually or in small friendship groups to enable them to live their lives as independently as possible.
Our services include support for people who manage with very few hours of support each week and people with higher level needs including people who require continuous staffing from 2 support workers.
At Social Care Solutions we have been able to make supported living a reality for people with the highest levels of need. We focus on enhancing life and social skills for independent living, assisting our service users to make appropriate choices and encouraging them to pursue their goals and realise opportunities for education and employment. We also encourage social inclusion and activities. This can include participation in sports and social events and also solo, group or escorted travel.
Central to our approach is an individual person-centred support plan. This tailored package of support is agreed with the individual and includes consultation with their family and any professionals who support them. It is carefully structured to provide exceptional personal support together with positive aspiration-based personal development.
Our support plans are regularly reviewed to ensure that an appropriate level of care and support is always provided.
