£16,048 – Full time contract (38 hours p/w)

We are looking to recruit a full time, fixed hours Support Worker for our Floating Support Service based in Enfield. We provide a floating support service to up to 40 adult service users, many of which live independently in the community. The individuals we will be supporting as part of this Floating Support Service have mild or moderate learning disabilities, with some being diagnosed with autism or high functioning autism (formerly known as Asperger’s Syndrome).

The key goals of the service is to promote community engagement, choice and independence in a planned, safe and person centred way for our service users, whereby individuals will receive support to play a more active and meaningful part in society. It is also the intention that some individuals accessing the service will only do so for a limited amount of time, during which they will be supported to achieve their own positive outcomes, goals and ambitions, such as:

• Achieving economic well-being and support with benefits; support with budgeting and maintaining tenancies.

• Accessing the community independently, and developing daily living skills.

• Finding a job and other meaningful activities

• Maintaining good social networks and preventing isolation

• Living in their homes as independently and safely as possible

• Exploring their cultural identity and needs

• Managing health needs; attending appointments and liaising with health professionals

• Support to set up and maintain links with other support services (Child care services; Enablement services; employment services)

We are also recruiting support workers to work in our Supported living scheme in Enfield. The scheme provides support to adults with learning disabilities, consisting of five houses and two Outreach customers. Most of our service users are diagnosed with a Learning Disability but ASD and Mental Health diagnoses are present as well. The services provides twenty four hour support, seven days a week, including sleep ins and some waking night cover. The Supporting Living service also provides domiciliary care to some of our service users, all of which have long term support needs.

As well as having an enthusiastic and positive attitude to supporting our service users achieve their goals and be as independent as possible, the successful candidates would be expected to do the following tasks as part of their role, and would ideally have relevant working experience of the following:

• Provide support to individuals in line with agreed support plans

• Coordinating packages of care and support for individuals (Key working); working with a caseload (Floating Support); supporting individuals with reviews of support and any needs for long term care or support.

• Good time management skills, showing  good judgement and commitment, in a lone-working role

• Demonstrate good IT and communication skills, and be able to record information clearly and professionally

• Good verbal communication skills; be able to liaise professionally with a range of external stakeholders, care managers, health professionals, other services and providers of support, and family members of service users.

• Delivering a service that promotes and supports choice, empowers individuals, helps people to take control of their own support and care, and increases their independence and self determination

• To put the service user at the heart of your work, promoting each individual’s needs for independence, well being and dignity

• Flexible approach to working shift patterns, which may include some evening and weekend hours, and bank holidays. (Supporting Living support worker role will be expected to meet these requirements, as well as being available to cover sleep in duties on a rota basis.)
