Job Title: Care Co-Ordinator (In-Take Team)

Location: Newcastle, NE1

Salary: £16 – £20K depending on experience and qualifications

Closing Date: 28^th November 2014

Hours: 37 hours per week

Essential: You will need to be a car driver, as you will be covering the North East area

Job Role:

An opportunity has arisen to appoint a Care Co-ordinator (In-Take Team) to join the company’s existing operations team in their Newcastle Branch.  The company are seeking applications from candidates with previous experience in adult and child care. 

The overall purpose of the role is to support the company’s client packages and provide their Health Care Assistants with mentoring support ensuring that the company’s teams provide a high quality service. As a Care Co-ordinator, you will assist the existing management team to provide home care in a caring and respectful way in keeping with all relevant Policies and Procedures and legal standards including CQC regulations.

Key Responsibilities:

• To action incoming referrals, liaising with the rotating team and CHC
• Carry out initial assessment of clients, developing a detailed Care Plan
• Build working relationships with internal and external stakeholders, including: CHC, district nurses, Occupational Therapists and clients families
• Providing on-call cover as required
• Using effective problem solving skills in order to develop our client packages effectively
• Prioritising and working to tight deadlines

Hours of work will be 37 per week, working flexibly over a 24/7 period where necessary.  You will be expected to take a flexible approach to your working day and will need to be prepared to work evening and weekend hours on occasions.
