Job Title: Complex Care Co-ordinator

Request provide nurse-led critical and complex care enabling clients to live as normal a life as possible in the comfort of their own home, among family and familiar surroundings.

Our team of nurses, specifically trained care workers, and support staff, provide a comprehensive range of services to support a wide variety of critical & complex needs. Some of the clients we support suffer from brain injuries, strokes, cancer, Parkinsons, MS, MND, Huntingtons, EB, cerebral palsy, Treacher-Collins Syndrome, advanced dementia and many more.

We work closely with local and national health care professionals and have pioneered quality home treatment for many high dependency critical care clients, who would normally have remained in hospital. Instead, with our support and clinical input, they receive similar care in their own home. Some examples of critical care we can deliver at home are:
• Tracheostomy care and changes
• Ventilation care
• PEG (gastric) feeding
• Bowel and bladder management

We have an exciting opportunity for a complex care coordinator to join their lively team based in Aldershot.

You will be responsible for looking after complex care packages which include clients and staff.
