An exciting opportunity has arisen for a Care Coordinator based in Bury St Edmunds, Suffolk to join an expanding company.
As a Care Coordinator your duties will be to:
• Be responsible for the effective roistering of staff resources to ensure contractual requirements are met and that the provision of care for clients is within companies the policies and procedures and meet National Care Standards and CQC requirements.
• Be responsible for ensuring that all care visits are delivered including arranging covering staff during periods of annual leave and sick leave.
• Carry out assessments of client’s individual needs and complete risk assessments of the environment and activities that are to be carried out during the provision of care.
• Develop partnerships and good working relations with statutory and voluntary agencies in order to achieve the delivery of high quality care that ensures that individual client needs are fully met.
• Be responsible for reviewing all visit log records and inform the registered manager of any issues or changes in client’s individual needs.
An ideal Care Coordinator will hold the following skills and experiences:
• Experience of financial recording (costing of care packages)
• Experience of managing complaints and compliments
• Full drivers licence and access to own transport
• Experience of producing staff rota’s
You will be working 9am - 5:30pm (Monday - Friday) Plus On call.
In return you will receive a salary of £19,000 - £21,000 per annum.
