This includes clinical interventions that will prevent hospital admissions and that will facilitate patients being cared for earlier in their recovery.

An exciting opportunity has arisen for a Band 5 Physiotherapist to join the Community Team based at Walnut Tree Hospital in Sudbury.

The successful candidate will manage a caseload of clients with a broad range of multiple and complex health needs, under the supervision of a Senior Clinician.

Duties will include:

To formulate an individual management/treatment plan for each client using clinical reasoning skills.

To ensure interventions are evaluated and treatment is modified in order to measure progress and ensure effectiveness.

To apply a high level of understanding of the effects of disability and provide training and advice to patients and carers on lifestyle changes.

To work with appropriate senior staff to effectively triage all referrals to the team.

To ensure all aspects of care offered by the team meet with agreed national, professional and local standards.

To provide advice to clients, carers and colleagues, as necessary

Hours will be 37.5 per week; Monday - Friday 8.30am - 4.30pm

Diploma or Degree in Physiotherapy

Evidence of skill relevant to the rehabilitation of adults with physical disabilities and the ability to demonstrate theory/practice link

A knowledge of primary and community services

A Knowledge and application of current best practice in the rehabilitation of adults with physical, sensory and cognitive disabilities
