We are looking to register GP Locums and Salaried GP's who are interested in working for many different clients

•Proof of current GP register (copy of GMC Certificate)
•Current CV
•Proof of Identification (copy of passport or UK driving licence)
•Confirmation of membership to MDU/MPS/Other professional insurance body
•2 References
•Confirmation of listing on English performers list
•PMETB / JCPTGP certificate
•Confirmation of Licence to practice
