Job Title: Self Care Facilitator
Location: Based in the Blackburn with Darwen area but will cover all areas of East Lancashire
Salary Grade: NJC points 23-25 (Starting Salary £20,603)

Our client’s vision is ‘Mental Wellbeing for All’. To assist in achieving this vision they are seeking a passionate and motivated individual for the position of Self Care Facilitator.

The successful candidate will have advanced skills and knowledge to achieve behavioural change in individuals. They will be able to prove their ability to successfully motivate lifestyle alterations that result in improved health and wellbeing. Working across GP surgeries with multi-agency teams the post-holder requires excellent communication and time management skills. The ability to speak Gujarati, Punjabi or Urdu is not essential but would be advantageous. The post will be based in the Blackburn with Darwen area but will cover all areas of East Lancashire.

This post is full time, will be a permanent contract and is subject to an enhanced Disclosure and Barring Service (DBS) check. With regard to the Self Care Facilitator position – the project is funded until 31 March 2016 a review of this position will take place in advance.
